MDU satisfaction survey

  1. The promoter of this prize draw is The Medical Defence Union Limited (MDU) of One Canada Square, London, United Kingdom, E14 5GS.
  2. The prize draw is open to anyone who completes an eligible satisfaction survey and tick the box in the survey to indicate they want to be included in the prize draw.
  3. Eligible surveys are those that specifically reference the John Lewis gift card prize draw.
  4. The draw is free to enter, and no purchase is necessary but see 2 above.
  5. All entries must be submitted via the satisfaction survey and only one entry per completed survey per person is permitted.
  6. The opening date for entries is 00:00 on 1 April 2025.
  7. The closing date of the prize draw is 23:59 on 31 December 2025.
  8. Entries received after this time will not be considered
  9. The MDU accepts no responsibility for entries not successfully completed due to a technical fault of any kind.
  10. The MDU accepts no responsibility for any purchases or memberships which are not completed for any reason.
  11. Five winners will be chosen by random draw performed by a computer process within 14 days of the closing date.
  12. The winners will receive one of five £100 John Lewis gift cards.
  13. The winner(s) will be notified by email or telephone (using details provided at entry) within 15 days of the draw. If a winner does not respond to the MDU within 30 days of being notified by the MDU, then the winner's prize will be forfeited and the MDU will be entitled to select another winner in accordance with the process described above.
  14. The prize(s) will be sent to the winner by email within 30 days of response.
  15. The prize for each winner is non-exchangeable, non-transferable and no cash alternative is offered.
  16. The MDU reserves the right to replace the prize with an alternative prize of equal or higher value if circumstances beyond the MDU’s control makes it necessary to do so.
  17. The decision of the MDU regarding any aspect of the prize draw is final and binding and no correspondence will be entered into about it.
  18. The MDU must either publish or make available information that indicates that a valid award took place. To comply with this obligation the MDU will send the surname and county of the prize winners and, if applicable copies of their winning entries, to anyone who emails website@themdu.com or writes to MDU Services Limited, marketing department, One Canada Square, London, E14 5GS enclosing a self-addressed envelope) one month of the closing date stated in condition 5 above. If you object to any or all of your surname, county and winning entry being published or made available, please contact the MDU at website@themdu.com. In such circumstances, the MDU must still provide the information and winning entry to the Advertising Standards Authority on request. Responses may be delayed if a winner has not responded within the agreed timescales.
  19. Participants are deemed to have accepted and agreed to be bound by these terms and conditions upon entry. The MDU reserves the right to refuse entry or refuse to award the prize to anyone in breach of these terms and conditions.
  20. The MDU reserves the right to hold void, cancel, suspend, or amend the promotion where it becomes necessary to do so.
  21. In so far as is permitted by law, the MDU, its agents or distributors will not in any circumstances be responsible or liable to compensate the winner or accept any liability for any loss, damage, personal injury or death occurring as a result of taking up the prize except where it is caused by the negligence of the MDU, its agents or distributors or that of their employees. Your statutory rights are not affected.
  22. Personal data supplied by you during this promotion will only be processed as set out in the MDU privacy policy. See also condition 14 above in relation to the announcement of winners.
  23. These terms and conditions and this promotion are governed by English law. If any entrants to this promotion wish to take court proceedings, then they must do this within the courts in the United Kingdom.
